Trigeminal Neuralgia

Trigeminal Neuralgia TreatmentTrigeminal Neuralgia

Trigeminal neuralgia is a neuropathic disorder in which there is a severe shooting pain in the face caused by the trigeminal nerve, which is located in the head. The function of the nerve is to pass signals of pressure, temperature, touch, and pain to the brain. The signals or sensations of the face, jaws, gums, forehead and eyes (surrounding area) are sent from the trigeminal nerve to the brain. It may begin as short pain pulse later it progresses to mild attack causing frequent and longer turns of severe pains. It affects women more than men. People aged 50 years or more are affected by Trigeminal neuralgia. Treatments for this condition include medicines, injections, surgery and alternative therapies.

What is Trigeminal Neuralgia?

Other names for Trigeminal neuralgia are tic doloureux, Fothergill’s disease or prosopalgia. It is a chronic facial pain condition which affects the fifth cranial nerve (trigeminal) in the head. It happens due to nerve injury or nerve lesion. The pain is very intense and can originate from a variety of locations on one side of the face and rarely on both sides in the same person at one instance. Incidents of bilateral pain events are very rare. The pain may involve the front of the ear, the eye and the area around it, lips, nose, scalp, forehead, cheeks, mouth, and jaw. Mild stimulation of the face can trigger a jolt of excruciating pain because the trigeminal nerve carries the sensation from the face to the brain. Pain can arise either during brushing the teeth, while putting makeup, touching the face, exercising, swallowing, or feeling a slight breeze.

Trigeminal Neuralgia Causes

  • The various causes of Trigeminal Neuralgia can be Compressed blood vessel on the nerve near the brain stem. Constant rubbing of blood vessels with trigeminal nerve root damages protective sheath i.e. myelin sheath around the nerve. This results in nerve irritation.
  • TN occurs in patients with multiple sclerosis.
  • Nerve compression from a tumor
  • Arteriovenous malformation in which arteries and veins are entangled
  • Injury to the trigeminal nerve during sinus surgery, oral surgery, stroke, or facial trauma

Symptoms of Trigeminal Neuralgia

  • Sudden, electric shock-like, stabbing facial pain lasting for a few seconds to a couple of minutes. These short duration episodes occur in quick succession and last for about a couple of hours. It may vanish for a while and may reoccur, maybe after months or even years.
  • Intense pain around the lips, or eyes, nose, scalp, and forehead and anywhere on the face

Diagnosis Of Trigeminal Neuralgia

The condition is diagnosed based on symptoms or the patient’s description of the pain. Trigeminal Neuralgia is diagnosed with Magnetic resonance imaging which determines or confirms whether a tumor or multiple sclerosis is a cause of irritation to the trigeminal nerve. There are no tests to determine the presence of TN.

How Trigeminal Neuralgia is treated?

The medicines for Trigeminal Neuralgia are certain antiseizure drugs like Tegretol/carbamazepine or Neurontin /gabapentin. Doctors may recommend a combination of Klonopin /clonazepam and Depakote /divalproex sodium to achieve pain relief. Even few antidepressant drugs are found to be useful in relieving pain.

Neurosurgery is recommended if none of the medications are able to relieve facial pain or reduce nerve sensitivity. Some therapies such as chiropractic, meditation, acupuncture and self hypnosis are found to be useful in reducing pain.
